Why it happens (and it is not lying)
Remember the core idea: a generative AI does not look up facts or understand them. It predicts what words most plausibly come next, based on patterns it learned from huge amounts of writing. It has no built-in way to check whether something is true; it only produces what is plausible.
So what happens when it does not have a clear pattern for the real answer? It does not stop and say "I do not know." It does what it always does: it generates something that fits the shape of an answer. The result can look completely reasonable and be entirely made up. People call this a hallucination. The AI is not lying, because lying means knowing the truth and hiding it. It genuinely has no idea it is wrong. It is filling a blank with something plausible, exactly as designed.
